Registered Nurse / LPN
We are currently seeking a Registered Nurse / LPN to join our team of dedicated employees.
Work Experience: Preferably in Mental Health experience RN/ MHPs, Behavioral Health and Nursing orientation. Current nursing license required with preference for Washington State. Individual must have sound physical and mental health and be willing to work with others in a cooperative and respectful manner. CPR and First Aid training required. Additional training will be provided or arranged upon hire. This includes Mental Health, Dementia, and Developmental Disability Specialty trainings.
Orientation: A work orientation period, with pay, for the RN/LPN will be provided. This orientation period will place emphasis on understanding the principles of Upriver Place ESF guidelines, the importance of supporting the residents in maintaining their personal dignity, independence, individuality, privacy, choice, and decision-making abilities. The training period will also address the standard of professionalism necessary in the delivery of services to residents. RN/LPN will be oriented to the multi-disciplinary team approach to care provided within Upriver Place as it will be their role to attend and manage the physical health or medical needs of the residents.
General Conditions of Employment: A pleasant, positive, quiet and mature attitude will be required to prevent anxiety or insecurity in the residents, as well as a high degree of professionalism in order to direct the caregiving staff (NAC, NAR, HCA) and work collaboratively with the other licensed nursing staff and MHPs in the performance of their duties. A high standard of neatness and personal cleanliness is required. Position will require workdays as scheduled by owner/administrator to comply with 24-hour on-floor nursing service(s) required of the ESF. Duties will vary according to shifts and schedules and may be adjusted or revised from owner/administrator to implement any changes that may affect Upriver Place policy and/or the quality of resident care.
Responsibilities:
To maintain the highest standards of professional care expected by both the facility and in accordance with nursing license standards.
To comply with Washington State regulations as required for both ESF and respective nursing license.
To exhibit administrative and interpersonal skills necessary for satisfactory job performance and coordination while working on a multi-disciplinary team.
To orientate new staff in the principles of Enhanced Service Facility guidelines, and to assist all staff in the performance of their duties.
To perform regular assessments on residents as required and necessary, and to perform pre-assessments on prospective residents when directed by owner/administrator.
When assisting residents in the facility, ensure all safety measure will be observed and followed per care plan (Person Centered Service Plan and Comprehensive Assessment) and Behavioral Support plan for each resident.
To develop and maintain a professional working relationship with residents, nurses, physicians, mental health professionals, and keeping them well informed of residents’ conditions and progress as needed.
To schedule appointments for behavioral health, physician visits, podiatry care, hearing tests, etc. as necessary or needed.
To supervise and assist with medication service, including medication administration, and to order medication from pharmacy. Nurse will be expected to communicate and coordinate any concerns or needs related to medications with the respective provider.
To notify next-of-kin or responsible person when there are changes in residents’ condition, accident or incident.
To respond to resident calls for health care services and to ascertain that residents’ health care needs are being met.
To complete routine assessments on each resident regarding their condition and medication service.
To coordinate dietary needs and communication with contracted kitchen for individual resident needs.
To supervise care staff to complete daily housekeeping and personal care needs and assist as needed.
To complete all license and job required continuing education and facilitate care giving staff to complete CEU's required.
To facilitate and/or coordinate group activities five times per week with the collective interest of the resident group.
To engage residents in independent or self-directed activities that are appropriate to the ESF setting, consistent with the resident assessed interest, choices, functional ability, preferences and individual care plan.
